You can change your name legally through a court process. You file a form called Petition for Name Change (linked below) in the county where you live. The District Judge in the county looks over your Petition. If the Judge okays (grants) your Petition, your name is changed.
Once your name is changed, it is your responsibility to tell everyone that needs to know. Make sure you think about all the legal, public and private agencies, companies and other people who need to know.
These forms can be used to ask the court to enforce a court order when the other person in your divorce or custody case is not following the order.
Note: In some places in Kentucky, landlords and tenants have certain rights that are not available in other parts of Kentucky, through the Uniform Residential Landlord-Tenant Act (“URLTA”). Be aware that, at this time, the only places in AppalReD’s service area that have adopted these rules are Barbourville and Pulaski County. For the rest of AppalReD’s service area, the following
rules will apply.
